Job Description
Primary function is to facilitate the referral intake process and coordinate care with the interdisciplinary team, the patient, their family, and the referral source. Responds to Agency and patient needs in a professional and creative manner. Receives and appropriately documents new referrals in a timely manner. HOSPICE AND HOME HEALTH SCHEDULINGINTAKE EXPERIENCE REQUIRED.*. Having the necessary qualifications and experience is important for this position.
